Overview
Client questionnaires are AML data-collection forms sent to individual contacts through the client portal. Each questionnaire follows a four-stage workflow: Not Sent, Sent, Submitted, and Reviewed. You manage questionnaires from the Questionnaires tab on a risk assessment detail page.
Questionnaire Statuses
- Not Sent — the questionnaire has been created but not yet delivered to the contact
- Sent — the contact has been notified and can complete the questionnaire in the portal
- Submitted — the contact has completed and submitted their responses
- Reviewed — a staff member has reviewed the submitted responses and approved them
Viewing Questionnaires
Open a risk assessment and select the Questionnaires tab. The table shows all questionnaires for that assessment with the following columns:
- Contact — the contact's full name
- Status — the current stage badge
- Sent — when the questionnaire was sent
- Submitted — when the contact submitted their responses
- Reviewed — when a staff member reviewed the responses
Use the Search contacts... field to filter by name, or the Status filter to show only questionnaires at a particular stage.
Sending a Questionnaire

Find the questionnaire row with a status of Not Sent.

Select the Send button in the actions column. The questionnaire is delivered to the contact's portal account and the status changes to Sent.
A toast confirms the action with the message "Questionnaire sent to client portal". While the questionnaire is in Sent status, the actions column displays "Awaiting response".
Reviewing a Submitted Questionnaire
When a contact completes the questionnaire in the portal, the status changes to Submitted.

Select the View button to review the contact's responses.

Select the Reviewed button to approve the questionnaire. The status changes to Reviewed and a timestamp is recorded.
You can view a reviewed questionnaire at any time by selecting the View button that remains available after review.
AML Profiles
Questionnaire responses automatically populate the AML Profiles tab on the same risk assessment. Fields that were filled in by the contact through the questionnaire are marked with a Questionnaire badge. AML profile sections include:
- Personal Details — date of birth, nationality, country of birth, marital status
- Identity and Tax References — NI number, UTR, passport number, driving licence number, PAYE reference
- Bank Details — account number, sort code
- Employment — employer name, previous accountant
When a questionnaire has been sent but not yet submitted, fields that are expected from the contact display a "Pending from questionnaire" placeholder instead of an empty input.
You can edit any field directly on the AML profile and select Save Changes to persist your updates.
